Iloperidone (Fanapt)

  • Rarely used
  • Low EPS, Low dyslipidemia
  • Moderate weight gain
  • Potent α1 antagonism
    • α1 receptors on cortical glutamatergic pyramidal cells stimulate glutamate release which stimulates GABA interneurons that ultimately inhibits DA release in striatum
    • Blocking these receptors is similar to blocking 5HT2A receptors in terms of decreasing EPS
  • Potent α1 antagonism associated with sedation and orthostasis

  • Central α1 Receptor blockade (like with prazosin) may be beneficial for PTSD
  • Iloperidone exhibits dose-dependent QTc prolongation
